This happens when the driver doesn't complete the lap (e.g. if there is a 0% condition part on the car) or the lap isn't simulated quickly enough to be read back in. We'll be improving clarity of the feedback in the next days.
We can resolve the latter issue entirely without too much trouble, I expect to have that done tomorrow.
