Once a driver reaches lvl 20 in all stats (aside from experience, which to my knowledge is advanced via racing, not training?), is there any benefit to continuing to train them?  If not, and I have 2 maxed out drivers, can I just sign the lowest rated/cheapest possible doctor, since I don’t need the advanced training/recovery efficiency and can save around $1mil/race?

And along the lines of driver training, if I have a talent 20 driver all maxed out, and I’m manager (or team?) level 20 too, but the driver is only 4.5 stars, how does he get to 5 stars?  I have had all-20 drivers with 18/19 experience and they are still 4.5 star.  Will 20 experience rating give that extra 1/2 star?

Yes, once all stats are trained up (or down for weight) it makes no sense to continue training. Experience is indeed trained by racing only. Apart from hiring a cheap doctor there's also the possibility to not renew the contract, once the contract expires the game assigns a low level doctor with 50k wage. With long remaining contract it's often better to find a cheaper one first though.

It happens often that drivers cap out at level 18 or 19 depending on their experience after finishing training. If training is finished no further level calculation takes place and thus any experience gained afterwards won't count towards level. As only skills count for pace it doesn't matter. Higher level just raises auction price and starting wage. To get a level 20 driver one skill has to remain at 19 until experience reaches 20.

Thanks for the detailed reply!  That all makes sense, I’ll have to keep an eye on the driver skill levels during future driver trainings.  I had been doing the “don’t renew contract” thing to get a 50k doctor already, and my current expensive 5 star doctor is up in 2 races so I just wanted to make sure I was understanding the system correctly and that there is no point in keeping him if my current drivers are maxed.
