I thought 5 star doctors removed 10 health per training session, but with the ones I've bought it seems you lose 11 just as you would with a 4.5 star. Am I just unlucky or is this how it actually is?

This might be because the doctor is level 19 and you are level 20.
I think there is the same problem with other staff but the other way around:
If you are level 18, you buy a level 18 but 4.5 stars CD, you get the same amount of DP as a 5 stars CD.

That's a known problem. For example, I reported months ago that I have a lv 20 doctor and it decreases by 11.

This is a known problem.. the doctor is the only staff member who works according to the stars.
The definition of doctor is clear: "decreases by 1% for each star".
A doctor of level 19, 5 stars decreases health by 11% and not 10% as it should.
This also happens with a doctor of level 20 , 5 stars, the health decreases always of 11% and not of 10%.
This only happens for level 20 managers with a level 19 doctor, 5 stars is a level 20 doctor, 5 stars.
So between a level 19/20 doctor, 5 stars, and a level 19 doctor, 4.5 stars there is no difference.
I forwarded this to the devs, a fix will be made as soon as possible.
